INCLUDE REQUIRED REGISTRATION FORMS & FEES PAYMENT WITH THIS FORM. FEES PAYMENT & SIGNED  FORMS MUST BE POSTMARKED (or paid with credit card) AT LEAST FIFTEEN (15) DAYS PRIOR TO INSPECTION DATE TO AVOID ADDITIONAL RUSH ENTRY FEES ($50 per horse).

 

Inspections are benefits for ONLY AWS registered horse/sport ponies. Owners of inspected horses/ponies must be current AWS members.  Contact organizer for stabling information, facility requirements (i.e. Coggins) and directions.

I, the undersigned, understand that this is a high risk activity and that I am participating at my own risk. I assume all risk and responsibility, and release the inspection facility and the American Warmblood Society, members, owners, agents. partners, trainers, managers, families, and/or friends and guests, my horse/pony(s) or other property from any cause whatsoever. I further agree to hold harmless each and every person from any claim, which might arise by virtue of injury to my horse/pony(s) or me or caused by horse/pony(s), family, guests, and/or me.  The AWS reserves the right to refuse service to anyone. I have read the AWS Inspection & Breeders Guidelines (Inspection Requirements). No refunds. Credits may be applied.

Send required registration application, Inspection entry form and all applicable fees to the AWS national office. Applicants must also notify organizer. A maximum of 30 horse/sport ponies and a minimum of 12 newly registered horse/sport ponies are required or the inspection may be cancelled. If applicable, bring “Pending” certificate to Inspection and give to AWS personnel. USA funds only.
